The forms provided here may be used to increase the interactivity of your web pages.  

The forms are designed so you can save each one to your computer, edit the HTML in the form to reflect the needs of your school and upload each one to your web site for greater interactivity.

Feedback Forms

 

The following list of forms has been designed so you can test out how they function and choose those forms which you can readily use on your school website.  

When you select a form, you will see a section at the bottom of each form where you may fill in your email address to see:  

  • how each of the forms functions 
  • what response each form gives to the person who fills out the form 
and when you collect your email you will be able to see:  
  • what details are sent from each form to the person collecting the information. 
If you decide to use a feedback form on your school web site, simply save the form to your local computer, edit the form to reflect your school details, trim off the part of the form indicated between the "cut here" comments, fill in the correct recipient details and upload the page to your school web site.  


All these pages must be placed on an internet server or they will not function. The ability to run the pages is restricted to your server space on SINA, so this means the pages cannot be run from machines on an internal network. Therefore, to test your pages, you must load them into the www directory of your school website. 

People from around the world will be able to fill in your forms pages as long as they are stored in the www directory of your school website.

All of these forms can be modified to achieve almost any emailed response or report that you require. Some forms are designed so the user must fill in required information, or they will not work. This can be turned on or off as appropriate in each form.   Use this form as a template to enable feedback as email.
Simply insert this information into your page.
You only need fill in the particular details of the person who will receive the mail.
Note that this uses a script on the Schoolsnet server.
Do not change the address for that script.
